- Responsible for providing nursing care through all phases of care taking into consideration age and developmental needs.
- Collaborate and support plan of care as determined by the physician and healthcare team; treat patients accordingly as approved by the physician. Performs periodic re-evaluation of the patient as necessary and makes adjustments in the patient’s treatment program.
- Administers medications according to established medication administration principles, physician orders and facility policy and procedures.
- Reviews, interprets, transcribes and carries out physician orders.
- Perform on-going periodic system assessment and data collection in a systematic manner, focusing on: psychological, psychosocial, cultural spiritual and cognitive status; nutrition; pain; patient/family education; family involvement and patient advocacy; patient and patient’s equipment needs.
- Assesses plans, implements and evaluates the care provided to patient assigned.
- Recognizes changing patient conditions and emergency situations and appropriately intervenes.
- Demonstrates critical thinking skills, sound judgment, and leadership in planning, implementing and evaluating the care of patient.
- Displays a working knowledge of disease pathophysiology, common diagnostic tests and procedures, as well as treatment plans.
- Provides direct care, to patients with a wide variety of diagnoses, conditions, and diseases through ongoing contact throughout the assignment/shift.
- Complete, accepts only assignments as requested for which qualified. May accepts and assigns in rotational home care immersion based on assignment length/time as requested.
- Provides legible, clear written or electronic documentation of all patient care provided, medications administered, and the patient’s response to care.
- Maintains confidentiality of all patient information in accordance with SmartMed
- Supervises professional and non-professional personnel providing patient care. Provides supervision to others through task assignments, direction, evaluation and feedback of assigned tasks.
- Able to perform any skilled nursing procedures.
- Completes mandatory SmartMed in-services and provides requested documents to keep employee file current.
- Educates and instructs the patient, patient’s family and other staff as required.
- Notifies patient’s physician and responsible staff of significant changes in a timely manner.
- Effectively plans, organizes, sets priorities and completes work assignments with minimum direct supervision per agency policies; within established time frames.
- Performs procedures, taught by supervising staff within the scope of their certification.
- Assists, performs, and trains patient/client and family in activities of daily living such as bathing, grooming, nail, and skin care, oral hygiene, toileting and elimination and adequate nutrional intake.

- Utilizes infection prevention and control measures such as universal precautions, handwashing and personal protective equipment.
- Maintains and prepare medical bags before daily visits.
